Endoscopic Treatment of Gastrointestinal Bleedings

چکیده
Digestive endoscopy represents an essential diagnostic and curative tool used when presented with a suspicion of gastrointestinal (GI) bleeding. Its role is not only confined to primary detection but also to establishing the severity of a lesion and providing relevant information regarding its risk of bleeding. New endoscopic techniques, accessories and compounds make the minimally invasive treatment suitable for a vast array of lesion types. Our main goal in this chapter is to summarize the main presentation of lesions at risk of bleeding, briefly classify them according to the gastrointestinal segment and finally detail the endoscopic treatment options currently available. Therapeutic endoscopy significantly reduced mortality from upper gastrointestinal bleeding. Most common causes of lower gastrointestinal bleeding are: colon cancer, diverticulosis, colon polyps, inflammatory bowel disease, hemorrhoids, anal fissures and angiodysplasia. Digestive bleeding diagnosis strategy requires rapid clarification of the following elements: recognition of gastrointestinal bleeding, specifying the location of bleeding, assessment of its severity, specification of the etiology of bleeding and patient’s risk factors. Early diagnosis of the cause of bleeding and endoscopic hemostasis reduced the need for hospitalization, blood transfusions and emergency surgery.

Dieulafoy lesion should be considered in massive gastrointestinal bleeding that may be difficult to localize. If the endoscopic and angiographic approaches fail, surgery must be considered according to the patient’s clinical condition within an appropriate time. Although mostly seen in the stomach of old male patients with co-morbidities, here we presented a Dieulafoy lesion in the jejunum of a...

متن کاملRectal Dieulafoy Lesions: A Rare Etiology of Chronic Lower Gastrointestinal Bleeding
Dieulafoy lesion is rarely seen, yet it can be life-threatening. This lesion makes up to 1-2% of gastrointestinal bleedings and must definitely be considered in gastrointestinal bleedings whose source cannot be identified. In this case study, the 75-year-old woman was suffering from active, fresh, and massive rectal bleeding. Colonoscopy was applied in order to find out the source of bleeding. ...
متن کاملAmeboma: an unusual cause of gastrointestinal bleeding during severe leptospirosis
BACKGROUND Severe leptospirosis occurs mainly in a tropical environment and includes icterus, acute renal failure and hemorrhages. These bleedings, which are mainly a consequence of acute homeostatic disturbances, can also reveal simultaneous diseases. Coinfections with other tropical diseases have been previously reported during leptospirosis. To our knowledge, invasive amebiasis, which can in...

Gastrointestinal bleeding (GB) due to angiodysplasias can cause severe, recurrent bleeding, especially in elderly patients. Angiodysplastic bleedings in the gastrointestinal tract have been associated with aortic stenosis and, more recently, hypertrophic obstructive cardiomyopathy, caused by an acquired coagulopathy known as Heyde's syndrome. Multiple factors are involved in the pathogenesis of...
